"what do you think?" izzie asked the interns. the four of them were waiting outside the chiefs office because meredith was in a meeting about a patient with burke.
"fifty says meredith gets tossed out on her ass and burke walks away clean." christina answered.
"i don't think the chief will be that harsh. she did report it, even if it wasn't immediately." madelyn chimed in.
"please be nice to her." george added.
the group turned when meredith walked out of the chief office.
"burke saves my ass in there." she whispered. christina let out a sigh, know she lost fifty bucks but also a sigh of relief that her friend wasn't fired.
"don't you all have something better to do?come on people. move!" bailey scolded the group. the all scattered away.
madelyn checked the time. it was almost three.
"shit. shit. shit. shit." she cursed. she was going to be late to the appointment with the o.b.
conveniently enough the o.b. was in the same hospital as her, she she quickly rushed over to the peds floor.
"dr grey. just on time." her o.b. said, leading her towards the patient room and laid her down on the bed, lifting her shirt and applying the cold gel to her womb, which had grown a bit in the past few months.
"you are three months, correct?"
"yeah."
"so there is one of your babies." the doctor said, pointing to the fetal monitor.
"and there's the second one. and there is the cord wrapped around its neck."
"what can we do about it? surgery? there has to be something."
"well currently, there is a lack of fetal surgeons, so it would be hard to get a surgery. we could carry the baby to term and see if it lives, we could abort the single baby through a c-section, or see how long you can carry the baby and when the cord gets too much, we take the single baby out and let you carry the other to term."

"oh, god. i need alex." madelyn cried.
"alex is the father?"
"yes. where the fuck is he? he said he would come." madelyn whined, taking out her phone to call him.
"hi. you've reached the voice mail box of alex karev. leave a message or whatever." his answering machine played.
"are you serious?" madelyn complained.
"can i call you later once i've talked to him and make a decision. and can you call a fetal surgeon, someone, that may be about to help me please. i would like to carry two babies to term." madelyn answered, sitting up from the hospital bed and walking out of the room.

"dr shepherd." madelyn said, her and meredith catching derek as he walked out of a patient meeting.
"mr levangie, the parkinson's patient. is he a good candidate for dbs?" madelyn asked.
"yes, but he's not interested." derek answered.
"okay, but i think it's worth talking to him again, pushing him." meredith chimed in.
"we're talking about a brain surgery that is performed while the patient is wide awake. there is a risk of paralysis, a risk of death, and the patient doesn't want it. it is not my job to push him into anything, and it's definitely not yours."
"okay.." meredith answered.
"since you are clearly uncomfortable with my decision it's probably best that you meredith, don't scrub in."
"but-"
"it's a minor procedure. you won't be missed. madelyn, come with me."
meredith stalked off and derek looked at bailey who was standing behind madelyn.
"you know they call you the nazi." derek said.
"so i've heard." bailey answered. derek shook his head and walked off. madelyn followed suit.

madelyn followed derek to the scrub room and started to scrub.

"derek, i need your opinion again."

"you're pregnant?"
"with twins. one of the babies umbilical cord is wrapped around the others neck. if i don't do anything, one of my babies will die. if i do something, there's a chance they'll both live, but also a chance they'll both will die."
"and you don't know what to do."
"yeah, pretty much."
"you've talked to a alex?"
"i did. he think we caught it early enough they'll both be fine, but i have doubts."
"doubts? about what?"
"i feel like i shouldn't get the surgery and guarantee the other child's survival. but that means sacrificing my other child. i loved growing up and having meredith and i want
my children to have that too. but they have to survive being born first."

madelyn spotted meredith and christina sitting at a table outside and quickly made her way over and sat beside meredith.
"look, you got a brain. you got into this program. just because shepherd munched your cookies doesn't mean that you didn't deserve what you worked for."madelyn heard christina say as she sat down.
"but he's making me look bad. i have to end it."
"end what?" madelyn asked, joining the conversation.
"baileys silently punishing her for sleeping the shepherd." christina summed up.
"bailey caught you?" madelyn asked meredith.
"at the party." meredith answered.
"is it true you get to scrub in on the tumor?" alex asked, joining the table along with izzie.
"don't sit here." christina scoffed as alex sat at a different table.
"you get to scrub in? how psyched are you?" izzie marveled. madelyn was confused about what they were talking about.
"boy, i've missed a lot." she muttered to herself.
"on a scale of one to ecstatic, ecstatic." christina answered.
alex decided to come back and sat down across from madelyn.
"you know what i think? i think burke wants to get into her scrubs." said alex.
"unless he's already there." madelyn thought aloud. christina sent her a death glare and
turned back to alex.
"why are you here?" she asked alex.
"she's totally doing burke." madelyn whispered to izzie.
"he kicked me off that surgery for the same crap you pull everyday." alex explained.
"if i stuck this fork into your thigh, would i get in trouble?" said christina.
"not if you make it look like an accident." meredith said nonchalantly.

"one second." the girl on the other line said. there was a few beeping tones before another woman spoke.
"dr. whitman speaking."
"hi this is madelyn grey."
"ms. grey, hello. how can i help you?"
"i've made my decision."

However slowly, Nico realized that he might actually want to stay in this fantasy world. After all, he's had enough listening to Professor Paul's lecture on calculus- the swords and sorceries were just an added bonus.
