It's one thing to feel uncomfortable doing something and another thing to not stop yourself from getting into such situations.

Your solution is simple: if you don't feel good doing something, then don't do it. Don't play the game at all, and if this upsets your friends, then it is their problem—not yours.

But if they really want you to play with them, and you really want to play too, then you have to talk to them to let them know that you don't like doing things that make you uncomfortable. If they don't agree to your terms, you know what you have to do. The other way you can deal with this is to divert them all from Truth and Dare to some other game (like dumb Charades/Heads Up, etc). That way, they won't notice what you're trying to do and won't be upset either.

Remember, do not let people walk all over you. You have every right to leave something if it doesn't give you a good feeling.
