"Derek" Derek could hear Alex' voice
Derek woke up "How is she?"
"She's been brought to recovery" Alex smiled
Derek was smiling and jumped on his feet "I'm going to see her" He said
"We'll be there as soon as possible. Jo's in the bathroom" Alex said
Derek nodded and quickly made his way over to Meredith's room on the ICU. She was laying in the big hospital bed, the breathing tube still in her throat
"She's doing okay" Teddy told Derek as she entered the room "Her oxygen still isn't as good as I want it to be but it's improving" Teddy told him
"How are her kidneys?" Derek asked
"Looking good" Bailey answered as she walked in
Derek let go of a sigh in relief
"Sit down and page us when she's waking up, okay?" Teddy suggested and Derek nodded and took a seat.
...
"Hi" Meredith whispered when after all these hours, Teddy finally agreed with getting Meredith off the vent
"Hi, good to see you back" Derek said as he kissed her lips
"Good to breathe again" She weakly smiled, feeling exhausted
"Rest" Derek smiled and held her hand
"Hmm" She muttered before her eyes closed
"It's normal, right?" Derek asked Teddy who was writing something down in Meredith's chart
"Her oxygen is still low but we're running some more tests. Don't panic yet" Teddy told him and walked out of the room, leaving Derek and Meredith by themselves
"I told you not to die. You'll be fine" Cristina said as she hugged her person carefully

"Mommy, we drew you some drawings" Zola said as she handed the cards, letters and drawings to her mother
"Thank you guys" She smiled widely.
"So tell mommy about the disco party you had yesterday with aunt Amelia and uncle Link" Derek said
"We had so much fun mommy!" Bailey started explaining when Teddy walked in and looked at Meredith's stats.

"Sorry for interrupting guys, but I need to do a quick exam on your mom" He told the kids and placed her stethoscope on Meredith's chest "Okay, I'll be back soon so we can take another scan of your chest" Teddy told them with calmness in her voice.
Teddy walked out of the room and immediately pages Bailey and Webber.

"Okay, Meredith. Your lungs are scarring again, we now know it's because your body is rejecting the lungs. We're gonna start you on new meds so your body will hopefully stop fighting them." Teddy told her and Derek after they took a whole bunch of tests
Meredith started sobbing in Derek's arms and hoped all of this wasn't really happening.
All of this was awful, she just wanted to be healthy and to be at home with the kids, relaxing on the couch instead of having to worry about every single breath
"I love you" She whispered and squeezed closer into Derek's arms
"What do we tell the kids?" Derek asked
"That their mother is dying?" Meredith snapped but immediately regret it "I'm sorry, I shouldn't have.. oh god" She started sobbing
"It's okay" Derek whispered, cuddling close against her, not wanna miss a second of having his wife close to him
"We should let them come over. I wanna spend as much time with them as possible" She whispered
"You're not going to die. Do you hear me?" Derek asked her with a stern expression on his face
Meredith ignored him and got lost in her own thoughts, thinking about what she could possibly lose.

A few days later, Meredith's breathing became worse and it became clear that her lungs were failing. She was back in the same situation she was before the transplant. Her kids were laying around her with Derek holding her close in his arms, enjoying every minute they could.

Meredith felt weak, even worse than before. Her kidneys were failing because of the medication she got to keep her lungs going. She's been kept on dialysis a few days a week and had to stay at the hospital. Everyone of her friends was trying to find new lungs for her
"Damnit!" Cristina yelled as she smashed her paperwork on the table
"Nothing?" Alex asked
Cristina shook her head "It's not looking good" She admitted
"Fuck.. I need to talk to her" Alex said and stormed out of the room, leaving a confused Cristina
Alex knocked on Meredith's door
"Hi" He smiled as he came in
"Hi" She smiled back
"Where's Shep and the kids?" Alex asked
"Getting.. food" She smiled back at him
"Mer.. I need to tell you something" Alex sat down on the bed
"What's.. wrong?" She was fighting for every bit of air she could get
"Jo's pregnant" He smiled
Meredith gasped "Really?" She asked as she started tearing up
"Really" Alex smiled back at her
"Congrats" Meredith smiled but started coughing uncontrollably
"Mer? Please take some deep breaths" Alex asked as he helped her upright, placing an oxygen mask on her face
"What's going on?" Derek screamed as he entered the room and quickly rushed towards Meredith's side
"Stats are dropping" Alex yelled
"Her organs are shutting down" Cristina yelled as she ran into the room, followed by Teddy and Miranda
"Derek, grab the kids and wait in the hallway" Alex ordered
"No, not now! Please Mer"
"Derek go!" Alex yelled and focussed back on Meredith
Derek was waiting a bit further down the hallway, the kids crying in his arms
"Derek?" He heard Alex' voice
"Yeah?" He looked up
"We got her back. She's intubated but she's awake" Alex said, tears streaming down his eyes "Her organs are failing.. it won't be long-" Alex sighed
"Wanna go see mommy?" Derek asked the kids and they nodded "Mommy has a tube in her mouth, it helps her breathe. Mommy isn't doing well and.. mommy.. this will be the last time we'll see mommy" Derek told them, trying not to break down in front of the kids
"You said mommy would be okay!" Ellis yelled as she started crying and ran towards Meredith's room, quickly followed by Zola. Bailey stood there, waiting till his father would say that all of this was a mistake
"Come on bud. Let's tell mommy how much we love her" Derek stood up, holding Bailey's hand as they walked toward Meredith's room
They entered the room and saw Ellis and Zola clenching their mother, clutching on her hospital gown with their tiny fists.
"Mommy, are you gonna die?" Bailey asked as he climbed on top of the bed
Meredith looked Derek in the eyes before looking at Bailey and giving him a slight nod. She reached out her arm for Derek to join them.
He sat down on the bed and Ellis stubbornly changed from Meredith's left side, where Derek was sitting, to Meredith's right side.
"Ellie" Derek sighed as he grabbed her hand but she pulled away, wrapping her arms around Meredith instead
"We love you" Zola told her mother. Ellis' fingers were stroking her mother's skin.
"I'll miss you" Bailey whispered as Derek stroked Meredith's hair until the familiar sound of the machines started beeping in the room, showing that Meredith was gone.
"Mommy" Ellis whimpered as the family lay there, on the bed. Not wanting to move, not wanting to lose Meredith
"Derek!" Alex' voice was heard
"Derek, man! Wake up, Mer's out of surgery" Alex said again
Derek woke up, "what happened" He asked
"You fell asleep. Mer's out of surgery, Jo is in the restroom, we'll be there soon." Alex said and Derek ran to Meredith's ICU room, he didn't believe this was a dream but there she was, asleep in the ICU bed, her stats showing that she was doing good
"It was just a dream" Derek sighed
