The Combined Military Authority is what became what's known as "The Combine" which was a military despotism centered around Betelgeuse that took over after we glassed Terra and when control shifted due to the heavy combat in the TerraSol system.
They crushed any rebellion against them from DASS, BASS, everyone. Even hung some planet-crackers over Rigel to ensure compliance. Devoted everything to destroying the Omniqueens.

All right. So, Omniqueens spawn out in kind of 'spokes', sending out Young Queens to the next system. Once the Young Queen reaches Omniqueen she sends out more.
Now, during the Great Culling, a handful of Young Queens kept their heads down. See, they had a plan. They let the Terrans stomp the hell out of everyone, then stayed quiet.
At the time, only a few of the Imperium Immortals were still around. Most of them out at the Great Hellspace Rift AKA The Eye. Daxin was one. One of the Omniqueens managed to kidnapped him.

Now, at the time, we had no idea there were about a dozen just matured Omniqueens out there.
Then Daxin ripped the head off of one of them.
We all felt that. I mean, for ten thousand light years, hell, maybe through the entire Milky Way Galaxy, we felt the Omniqueen beg, felt her scream when Daxin ripped out her eye, then felt her die when he sawed her head off with a Warrior Caste bladearm.
That made all the other Omniqueens jump before they were ready.
What they didn't know is that the Terrans had installed High Frequency Phasic Control Inhibitors on just about every world.
See, the Omniqueens had intended on having mature queens enter Terran space, take control of the Mantid citizens, and attack the Republic of Aligned Planets from within.
To top it off, every Mantid in existence heard the roar.
Which led to the Great Rebellion.
